Who You Are

We are seeking an experienced Locum Adult Social Worker to support residents living in an Extra Care Housing scheme during a period of service change. The ideal candidate will be a qualified Social Worker registered with Social Work England, possessing recent experience in adult social care.

What the Job Involves

The post holder will undertake Care Act-compliant assessments and reviews to ensure appropriate care and support arrangements are in place during the transition away from an Extra Care Housing model. Responsibilities include:

  • Completing timely Care Act needs assessments and reviews.
  • Supporting residents through transitions, reflecting revised accommodations in care plans.
  • Collaborating with housing providers and health professionals to ensure care continuity.
  • Identifying and managing risks associated with care and accommodation changes.
  • Conducting Mental Capacity Assessments and Best Interest decision-making.
  • Keeping individuals, families, and advocates informed and supported.
  • Maintaining accurate case recording per statutory requirements.

Skills

  • Qualified Social Worker with Social Work England registration
  • Experience in adult social care, especially with older people or in housing-based support
  • Knowledge of the Care Act 2014 and Mental Capacity Act 2005
  • Experience in assessments and reviews within Extra Care or similar settings
  • Autonomy and confidence in managing a complex caseload
  • Experience in service transitions or provider changes (desirable)
  • Experience with partnership working with housing providers (desirable)
  • Knowledge of commissioning and care provider handover processes (desirable)
