What the Job Involves
As a Social Worker in the Newport Discharge Service, based in the Royal Gwent Hospital, you will be integral to facilitating and managing hospital discharges. You will coordinate with individuals, families, carers, and other stakeholders to ensure appropriate discharge arrangements are made. Your role will involve supporting and influencing discharge pathways and improving the overall hospital experience for patients. This on-site position offers limited opportunity for home working, subject to management approval.
