You are a dedicated individual passionate about supporting young people and care leavers aged 18-25. You possess a strong commitment to facilitating successful transitions into adulthood for those you support, particularly focusing on those with complex needs and vulnerabilities. You are well-versed in national legislation, policy, and procedures relevant to care leavers, and you are capable of building and maintaining professional relationships with them.
What the Job Involves
The role involves holding an allocated caseload to provide information, advice, and guidance for young people leaving care. You will coordinate and facilitate a multiagency approach to safeguard and support care leavers, perform assessments of need, and support the development and monitoring of Pathway Plans. Regular visits and maintaining contact with young people are essential, as well as advocating on their behalf and ensuring the provision of necessary services. The role requires managing the allocation of resources such as university bursaries and transition grants, assisting with options like education, employment, and housing, and actively engaging young people in planning their futures. Occasionally, travel within the UK and overseas may be required based on individual circumstances.
