Who You Are
You are a committed individual with a caring nature, capable of working in a dynamic and rapidly changing environment. You are flexible, focused, and possess strong observation and recording skills. You have the potential or are willing to train to achieve the relevant HSc level required for the position. Proficiency in additional languages, particularly Welsh and Community Languages, is an advantage. A full clean driving license and access to a car are required. This role is open to female applicants only.
What the Job Involves
As a member of the Community Resource Team (CRT) Homecare, you will support Cardiff citizens, helping to prevent unnecessary hospital admissions and facilitating discharges. Your goal is to assist individuals in living independently at home, working closely with health professionals to support users in achieving their reablement and wellbeing goals. You'll perform assessments and create plans tailored to individual needs. The job involves working shifts according to a rota, including potential evening and weekend work. Tasks are received via a mobile device, guided by a service delivery plan.
