Who You Are
You are a dedicated and compassionate professional, qualified to at least QCF Level 2 in Health & Social Care, and registered with Social Care Wales. You have a strong commitment to supporting individuals' independence and exercise an outcomes-focused approach with experience in working effectively within community or home care settings.
What the Job Involves
- Support complex cases to enhance clients' independence at home following illness or hospital stay.
- Line manage a team of Reablement Care Workers, ensuring quality service delivery and compliance with regulated standards.
- Monitor client cases, providing feedback to team leads and assessors for service appropriateness.
- Collaborate with team leads to ensure high service standards, meeting departmental policies and procedures.
- Encourage service user independence by empowering informed choice.
- Maintain high-standard records and mentor care workers in community settings.
Skills
- Experience in working with older people or individuals with disabilities
- Understanding of home care settings
- Line management and supervision experience
- Communication and teamwork skills
- Numerical and computer literacy, proficiency in Office 365
- Knowledge of CIW standards and Health and Safety
- Flexible and able to work varied hours if needed
- Full driving license with car access
