What the job involves
As a Short Breaks Practitioner in the Children and Young People’s Disability Service, you will be responsible for conducting high-quality assessments and reviews of care packages for children with severe disabilities and complex health needs. You will work closely with families, ensuring that support plans are in place to meet educational, health, and social care needs. Your role involves collaborating with a wide range of agencies to ensure coordinated delivery of services. You will focus on enhancing the involvement of children in decision-making processes, promoting their independence, and supporting families to build resilience. You will be instrumental in ensuring that services are accessible and inclusive of all communities within the borough.
Skills
- Knowledge of pressures faced by families in deprived areas
- Experience with children with disabilities in multicultural communities
- Understanding of legal frameworks - Children Act 1989, Children and Families Act 2014
- Assessment and care planning skills
- Proficient in Microsoft Office and IT systems
- Excellent communication skills
- Resourcefulness and innovation
- Risk identification and safeguarding capabilities
